Information required to complete the Request for Arbitration form Read the instructions for completing the form. Make sure you have copies of all documents and other evidence you want to provide when submitting the form. When you submit the “Request for Arbitration” form, you must attach copies of all required documents, records and repair orders. All registered owners of the vehicle must sign the form. You will notice that the FLRA has designed the forms in two versions – PDF and Word. The arbitration program is designed in such a way that a consumer does not need a lawyer for most claims. However, you still have the right to be represented by a lawyer on any claim. The manufacturer may be represented by a lawyer. Most major car, truck and motorcycle manufacturers do not have lawyers to represent them in arbitration. Note: “Reasonable” attorneys` fees will only be reimbursed to you in an arbitration award if the manufacturer has also been represented by a lawyer. Keep a copy of the request for arbitration for your records and send the original form to the Lemon Law Administration. If you are approaching the 30-month deadline to file a request for arbitration, send the form and documents by registered letter (acknowledgment of receipt requested) or by express delivery “overnight”, deliver it in person, send it by FAX or email it to lemon@atg.wa.gov.

Note: The date of submission of a request for arbitration will not be accepted as the date of “receipt”. On the Request for Arbitration form, you will be asked to indicate a preferred time for the hearing. Arbitration appointments may be requested for business days during office hours or one evening at locations across the state. IT IS VERY IMPORTANT TO TAKE THIS INTO ACCOUNT CAREFULLY. IT IS VERY DIFFICULT (AND PROBABLY WON`T BE POSSIBLE) TO MAKE CHANGES AT A LATER DATE. The Board of Arbitration will try to accommodate your schedule, but cannot guarantee that your hearing will be scheduled upon request or at a time that is most convenient for you. (2) Word version: Use this version to fill out the form in Word. After you save or copy the content of the form, you can delete sections that are not relevant to your particular exceptions or contradictions. You must send copies of repair orders for any diagnostic or repair attempts related to any defect in your claim.

If you are unable to provide the repair orders and other documents, you must explain the reason for the absence on a separate page attached to the Request for Arbitration form. If you have not received a repair order after an attempt to diagnose or repair a defect, there may be records, including mechanic`s notes and repair billing records in the after-sales service. Please note that you only need to fill in the parts of the forms that are relevant to your specific exceptions or objections. If you cannot find an application or form, please check under National Forms. If the form is not completed in its entirety or if the required documents are missing, your application cannot be sent to the New Motor Vehicle Arbitration Board. It will be returned to you with instructions on how to proceed. After completing the appropriate form, print it out and submit it to the Office of Admission and Case Publication. Fill out the form completely. When you complete the form, you must clearly describe each defect, indicate when each diagnosis or repair attempt took place, the mileage of your vehicle at the time of each attempt, the dealer who carried out the repairs and the number of days your vehicle was out of service due to a diagnosis or repair. If you have not completed the form, it will be rejected. Add additional statements or information that you believe relate directly to defects or other elements of your claim on additional pages. If you are submitting consent to the District Judge, please use the local form. If your request for arbitration has been filed, you will be notified that your claim has been assigned to a paralegal at the Lemon Law Administration. The paralegal will be available with other staff to answer your questions, provide you with information and help you prepare your submission of claims for the arbitration hearing. To encourage a quick resolution, the basic information in your request for arbitration form is immediately sent to the manufacturer by the Lemon Law Administration.

You and the manufacturer are invited to communicate the possibility of an agreement throughout the arbitration, even if previous negotiations have failed. The authority has created forms that serve as templates for filing exceptions to arbitral awards and oppositions to exceptions to arbitration. These forms are integrated into the FLRA e-filing system, which is the preferred method of registration. We also provide the forms listed below in case you want to fill them out with Adobe or Word and then upload them to the e-filing system. You should also use these forms if you choose a method other than electronic filing – mail, commercial delivery or delivery. .
